Upon completion of your study abroad or exchange program, you are responsible for making arrangements for the host institution to send an official transcript to 51�Թ�app's Registrar's Office.

Before you begin to fill in this form, make sure you have:

Please list the name/title of the proposed courses for transfer back to 51�Թ�app.

Indicate the credit value of each course at the host institution.
When possible, include a link to a course description or syllabus.
If there are no online course descriptions or syllabi available on our partner university’s website, you will need to request course information for all of the courses you have indicated on this form from the International Office at your host university.Once you have received course descriptions, please forward them to rwalker@mta.ca
Courses cannot be assessed for transfer credit without this information.
